Why is my Denon AVR s750h audio cutting out?

Audio returns once the receiver switches back to CBL/SAT input. This is starting to get really annoying. Spectrum DVR box was just swapped out about 1.5 years ago. 2) A less annoying issue, but still an annoyance, the audio sometimes cuts out randomly for just a quick second or so while watching live cable.
